Change of Caregiver/Provider
The parent/guardian must submit a change request in writing with the previous istock_000004248812preschoolersxsmallcaregiver’s name, the new caregiver’s name, the child(ren)’s name(s) and the date of the change.  The request must be signed, dated and returned to our office before the change can be made.  These requests must be written within thirty (30) days of the change.  If our office is not notified within this period of time, it will be the responsibility of the parent/guardian to pay the provider for services.
