Our monitoring workers perform unannounced visits to the providers to make sure the caregiver/ provider is abiding by all policies.  All areas of the home shall be accessible to monitoring staff of Choctaw Nation. Site visits are attempted for up to a total of three (3) attempts.  Every unsuccessful site visit shall be documented with a notice left at the door and the carbon copy kept on file.  After the third attempt the applicant or current provider shall be denied approval.
